... Facility stated that they will hire an extra staff member to cover breaks and will submit statement of correction no later than 5/8/25. LPA observed there were no additional staff available in case of emergency and Director was required to provide AM/PM breaks as there were no additional staff available which is a potential health, safety and personal rights risk to children in care.
… 1 Director stated she would provide S1 with personal rights training and also review videos on the CCLD website and provide a written statement and disciplinary note signed by staff member to be sent via e-mail to LPA Lane by 4/25/25. Based upon investigative interviews, four individuals stated they witnessed S1 aggressively placing an infant into a play yard and using inappropriate language on 1/8/25, which is an immediate health, safety and personal rights risk to children in care.
Based on information obtained from text messages, photos, and interviews with outside sources, it was determined that facility does not have an adequate amount of cribs for infants enrolled and will have infants share cribs or a play yard which is an immediate risk to the health, safety and personal rights risk to children in care.

This requirement was not met as evidence by… 1 Director will provide a written statement ensuring facility will be in accordance with the plan of operation including using only licensed areas for the children and a roster enrollment showing appropriate capacity to LPA Lane via e-mail by 4/29/25. Based upon investigative interviews and photos obtained, the facility was using an unlicensed space (the director’s office) to care for additional infants which is a potential health, safety and personal rights risk to children in care. 8
… OBSERVED CORRECTED TODAY. Coralito Garcia stated that she will make sure that they have sufficient number of cribs for the number of infants enrolled in the program. Currently there are 7 infants enrolled and the facility has 7 cribs in the room. Based upon information obtained from interviews with staff and parents, it was determined that the facility did not have an adequate amount of cribs for infants enrolled and put an infant in a crib before removing the other infant’s sheets which is a potential health, safety and personal rights risk to children in care.
... 1 Director stated she would provide a written plan to LPA via e-mail stating how the licensee will ensure that capacity is maintained at all times by 4/21/25. Based upon text messages from reporting party and interviews with staff and parents, it is determined that 5 out of 9 staff and 1 parent observed 10 infants in the infant classroom with is a potential health, safety and personal rights risk to children in care.
… 1 POC: Licensee will conduct a training with current staff members and any new staff members on ratio requirements for infants and submit a signed agenda with staff attendance sheet via e-mail to LPA Lane by 4/21/25. Based upon staff and parent interviews conducted, 6 out of 9 staff and 2 out of 4 parents stated that the facility was out of ratio repeatedly over the course of the past 6-7 months which is a potential health, safety and personal rights risk to children in care.
... 1 Director will provide updated LIC9227 form signed by provider and parent that C1 can roll over from back to stomach and stomach to back no later than 1/30/25 and submit to LPA Lane by e-mail: Keturah.Lane@dss.ca.gov. Director will hold a training for all infant staff to explain how to properly fill out the LIC9227 form in a timely manner as required by regulation and send staff agenda with signatures. Based upon LPA observation that C1 was sleeping on his stomach and record review of the child's LIC9227 Individual Infant Sleeping Plan was not updated to show that the child can roll over by the next business day is a potential health, safety and personal rights risk to children in care.
Based on records review, 5 of 5 children’s files were missing parent/guardian verification they received/reviewed Type A deficiency report the facility received on 10/27/23.
101227(a)(1) In child care centers providing meals to children...All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. 1 Director stated they will double the food ordered and provide proof of food ordered in accordance to the menu and enrollment and provide purchase order to the Department by 3/22/24 for the week of 3/18/24 -3/22/24. Based on interviews, observations, and documents reviewed investigation interviews support allegations, and purchase orders and delivery times show in February insuffienct food at the begining of the week which poses/posed a potential health, safety or personal rights risk to persons in care. 8
Immunizations: Prior to admission to a family day care home, children shall be immunized against diseases 1 Director has provided missing immunizations as of site visit. Deficiency is cleared. Based on records review 1 out of 5 children's files reviewed was missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care. 8
Based on the information obtained during interviews, observations, and videos it is determined that S1 violated infant C1's personal rights by handling the child in a rough manner.
Reporting Requirements...during the operation of the child care center of any of the events...report shall be made to the Department by telephone or fax within the Department's next working day….Any suspected physical or psychological abuse of any child. Director stated she will submit LIC 624 to the Department by POC date. Based on information gathered by video and interview, Facility faciltiy to reprot personal rights violation after obtaining video proof of the incident. 8
