In reviewing facility records LPA observed that child #1 had not been signed in since 7/11/2025 and was present during today's inspection. Per staff present, the change had been made about 3 months but never reported to the Riverside Child Care Regional Office, as required. This poses/posed a potential health, safety or personal rights risk to persons in care.
