Based on observation, the licensee did not comply with the section cited above. In conducting a physical inspection, LPA observed a classroom that was created by taking away space/square footage from the infant classroom. The classroom no longer has sufficient square footage for the licensed capacity. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.

In conducting a physical inspection and in reviewing the facility sketch, LPA observed a middle classroom that was created by taking away space/square footage from the infant classroom. The Department did not receive updated documentation an/or authorized the alterations. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. The napping area where two cribs are located, is not physically separated from the activity space. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. In reviewing children/infant records, child/infant #4 did not have an updated needs and services plan. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA record review, the licensee did not comply with the section cited above in count of 5 out of 5 infant records reviewed which pose a potential health, safety or personal rights risk to persons in care. The facility provided visual sleep checks, but did not conduct any documentation on them.
