Based on observation and interviews 2 staff were present but engaged in sweeping and arts/crafts so 4 children on rug area had no visual supervision for 5 minutes or less, and magnets were thrown causing an injury near C1’s eye. This poses a potential health, safety or personal rights risk to persons in care. The Director also had staff watch the Child Care Videos’ regarding care and supervision in a Child Care Center. The Director Director provided signed copies of training at the 4/7/22 visit so POC was cleared at visit.
Based on interviews and record review that C1's admission agreement was modified and the child was placed in a higher age group based on verbal discussion but with no signed/written addendum to the agreement. This poses a potential health, safety or personal rights risk to persons in care.
