Minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record. This requirement was not met by: Documentation missing from C1's file 1 Facility representative will train all staff to fill out incident reports to meet the requirement of CCR 101226(a)(2). Based on interviews, one child (C1) obtained injuries while in care that staff could not explain.
No child(ren) shall be left without the supervision of a teacher at any time... This requirement was not met as evidence by 1 Facility representative will create a plan of how they will prevent children being left in classroom and review it with staff by 1/19/26. Signed acknowlegdement will be sent to: emily,curiel@dss.ca.gov by 1/19/26 Based on information reported and interviews conducted the facility did not comply with the section cited above in regards to two children being without supervision. 8
