These files also did not contain an appointment date for the medical assessment of the children or a tuberculosis test within 30 days of the children's admission. Both of these facts combined poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed during record review the facility was not cited for a lead exceedance in 2022/2023. Director stated a retest of outlet identified with lead is being conducted. .
