the Department becoming aware that there were COVID positive cases at the facility for two staff and one child on 11/30/20, 12/3/20, and 12/20/20 that were not reported to the Department. This posed a potential health and safety risk to the children in care.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses an immediate health, safety or personal rights risk to persons in care.
... Director states that they will talk to teachers about not making the mask wearing optional and will speak to the children every day regarding mak wearing. Conversation will include explanation of the need for the mask and continue to encourage the mask wearing throughout the day. This will be implemented immediately. Based upon LPA’s and LPM’s observations and interviews with staff and children the citation previously issued has not been corrected. Children are not being encouraged to wear face coverings while indoors as required by California Department of Public health. This creates an unsafe environment at the facility, which poses an immediate health and safety risk to children in care. 8 A follow up visit will be made in the future to verify the correction.
... 1 Director stated she would send a video of the mask curriculum activities with the children and will e-mail that to LPA Lane at Keturah.Lane@dss.ca.gov and in additon provide staff meeting agenda (with signatures from attendees) for how to involve staff in long-term plan for encouraging children to wear masks. Based upon LPAs observations and interviews with staff and children the citation has not been corrected and children are not being encouraged to wear face coverings (required by DPH guidance) while indoors which is creating an unsafe environment for the children. ... 1 Director will send a written plan of action stating what steps the facility will conduct to ensure masks are worn indoors to maintain a safe environment. Plan should be measurable and state how it will be incorporated into the curriculum. Report must be posted for 30 days along with Notice of Site Visit.
