Based on interviews, a staff turned to attend another child, which resulted in C1 biting C2, which poses an potential health and safety risk to children in care. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Based on record review, the licensee did not comply with the section cited above in which staff members have an expired Mandated Reporter Training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

a child that left the preschool campus which poses an immediate Health and Safety risk to children in care. 8 staff will complete questionnaires and submit the original ink-signed and dated copies to the LPA. Quarterly reminders to parents will be sent to reinforce the safety measures required and not to let children other than their own leave through any gates.
