In rooms "Little Cubs", "Farm Friends" and "Jungle Explorers" the LPA observed drawers under the sink area to have keep out of reach items such as aerosol spray, Lysol spray, stapler's, etc that were accessible to children as the child proof latches were broken or not working. This poses a potential health, safety or personal rights risk to persons in care.

The LPA observed an outlet in the play yard, that was on the wall, that had exposed wires and the cover was not secured. This poses a potential health, safety or personal rights risk to persons in care.

Upon file review, the LPA observed two out of six children, did not have proof of TB test. This poses a potential health, safety or personal rights risk to persons in care.

LPA observation that children were not wearing face coverings during 1 LPA informed Director of the requirement per the latest COVID-19 Childcare Facility Guidance issued on 6/29/21. Director has agreed to submit a plan to Community Care Licensing outlining how facility will follow COVID-19 Childcare facility guidelines by 10/15/21. inside activities per CDPH guidelines for COVID-19 in Childcare facilities. Director stated they have been leaving it up to parent’s to decide whether their child should wear a face covering while in care. Staff were wearing face coverings but some were not worm properly as it was below their nose. 1 staff was wearing a face shield instead. This poses a potential risk to the Health, Safety, and Personal Rights of children in care.
