Buildings and Grounds - The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. 1 The Director stated that she will have the cabinet replaced or repaired and will ensure that there are no odors coming from the cabinet by the Plan of Correction due date, July 10, 2026. During the indoor inspection, LPA observed the area under one sink in the Orange classroom to have water damage with warped boards, discoloration and dark marks on the walls of the cabinet and there was a dark, powdery substance on the bottom of the cabinet under the pipes. LPA observed that there was a musty odor coming from under the sink and into the classroom. 8
Subsequent to initial licensure, a person specified in subdivision (b) who is not exempt from fingerprinting shall obtain either a criminal record clearance or an exemption...prior to employment, residence, or initial presence in the facility. 1 The two staff members left the facility. The Site Supervisor agreed to follow up with Guardian regarding the two staff members' clearances. The Site Supervisor agreed to submit proof that the issue was resolved and/or will have the two staff members re-livescan by the Plan of Correction due date, June 1, 2026. Based on observation and record review, it was found that two (2) staff members did not have the appropriate fingerprint clearance on record before working at the facility. This poses an immediate health, safety, or personal rights risk to children in care. 8
The Director stated that she will retrain staff on appropriate care and supervision and will submit proof of training/signatures of participation to the Community Care Licensing office by the Plan of Correction due date, 05/08/26. Per staff interviews, staff did not provide appropriate supervision during outdoor play, which resulted in a child becoming injured with a fractured arm. This poses a potential health, safety or personal rights risk to persons in care.

Child #1 pulled down child #2's pants and underwear and used a toy spatula inappropriately. Child #2 said, 'Teacher.' Staff #1 observed the inappropriate interaction. This is an immediated personal rights, health and safety risk to children in care. A civil penalty was issued. 8 Licensee will be appealing this citation. Based upon results of the appeal, licensee may change the protocol as needed.
