Supervision shall include visual observation. 1 Director Snowden stated she will conduct a staff meeting on 1/6/2025 regarding active supervision and will provide a sign-in sheet of attendees to Fresno Regional Office by 1/10/2025. Based upon staff interview, Staff left a child unsupervised in the restroom and did not realize it until a child brought it to staff attention. Due to lack of supervision this poses as an immediate risk to the health and safety of the children in care. 8

Director Snowden provided sign in sheets and minutes from meeting. Based upon staff interview, staff did not report of injury to child within the Department’s next working day and during its normal business hours. Due to lack of Reporting Requirement this poses as a potential risk to the health and safety of the children in care. 8

Buildings and Grounds: (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. During today's inspection of the play yard, LPAs observed the fence by classroom 4 to have two unraveled parts that have a sharp point poking out. 1 Director agrees to put in a work order to address all deficiences and will sumit pictures showing proof that deficiencies were addressed to LPA by due date 03/20/2024. LPAs observed holes where the sprinklers are located, the privacy slats on the fencing around the air conditioner was coming unraveled and laying on the ground, there were mushrooms growing from the ground near one of the play structures, and paint chipping on the exterior wall. In room 10, ceiling panels show water damage. 8
