Based on LPAs observation and interview, the licensee did not comply with the section cited above, the preschool playground had rabbit feces around the play area which poses a potential health, safety or personal rights risk to persons in care.
101212(b) Reporting Requirements(b) The name of the child care center director.... shall be reported to the Department within 10 days of a change of child care center director or designee(s) This requirement is met as evidence by 1 LPA received a written statement on 2/7/25 from Site Coordinator designating Jennifer House as the Center Director. Based on LPA’s interviews and record review, 3 out of 3 staff stated new director started in October 2024. Licensing Program Analyst (LPA) or the Community Care Licensing Office were not notified, which poses a potential health risk to the children in care. 8

LPA Duron interviewed Director, Jennifer House and she stated her start 1/13/25, and has been in the Director position since 2/3/25. As of 2/7/25 Orange County Community Care Licensing Regional Office or LPA Duron or LPA Chan did not receive notification for new director, Jennifer House at the time of visit. 8

Teacher-Child Ratio- There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance... This requirement is not met as evidence by: Based on LPA’s interviews, 5 out of 9 staff disclosed that classrooms have been over 1 Director stated she will also start staggering the aides' schedules to meet the children's needs. Director will email LPA POC plan by due date at dianna.valdezsantana@dss.ca.gov ratio. S1 admitted that being over ratio happens but not often. S5 stated they may have been over ratio only for a split second. S7 stated 99% they are okay, but during the transition when another staff is set to come it happens but not often. 8

Fixtures, Furniture, Equipment and Supplies: All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition... This requirement is not met as evidence by: 1 Director provided LPA with an invoice of a Plumbing company showing the faucet was fixed on 6/21/23. Based on LPA’s interviews and observations, S4 disclosed that the sink in room #3 has not been working since April 2023. S5 stated, It hasn’t been working for a month and there has been plumbers coming to fix it. Child #1 (C1) said the sink in the classroom has not been working for a little while. 8
Personnel Records: a)The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (11) A health screening... and (12) Tuberculosis test documents... 1 Director stated she understood that all new hires must provide all required health documents prior to working at the facility. Director stated she did have S10 complete the missing documents after LPA's visit. This requirement is not met as evidence by: S1 disclosed that S10 did not have on file her health requirements to work with children which was the health screening form or proof of TB test. 8
