(a) Each licensee shall have and keep on file a current written, definitive plan of operation... be submitted to the Department...(b) The plan and related materials shall contain the following: The Director stated, facility will update the facility's Transportation Permission Slip and state a children may need to wait at a designated location with a qualified teacher, and parents will be informed of designated area as needed. POC will be submitted via email on 3/28/25. This requirement is met as evidence by: Based on LPA’s interviews ,observation and record review. LPA reviewed Fun 4 Kids school age Enrollment Packet , Transportation contract, permission transport form, Admissions Policy , Application for Admission and School age daily schedule and did not observe documents stating enrolled children may need to wait at Bryant Ranch Park during pick up time which poses a potential health risk to the children in care. 8
Based on and interview conducted, it has been determined that the facility left 2 children unattended and with absence of supervision inside the vehicle. This poses an immediate risk to health and safety of persons in care
Director stated she hired 2 new staff and staggered the aides' schedule to meet the ratio needs. Director said she will have staff meeting today to address comingling and other urgent topics. Diirector will email LPA meeting agenda and staff sign in. Staff #3 stated, when over by one, S3 sends school-age child with preschool. Staff 8 there is comingling between, infants, preschool and school-age during 6am-7am. Teacher-Child Ratio: A teacher shall supervise no more than 14 children or with an aide a maximum of 28 children. This requirement is not met as evidence by: 1 Director stated she hired 2 new staff to help address the child-teacher ratio. I also have my teachers working over-time when needed.
