Based on interviews, it was confirmed that there was an absence of supervision as a child exited the classroom and was located in a gated outdoor area. 8 On 04/24/2026, Facility conducted a training regarding supervision in the classroom, playround, and bathroom. LPA was provided with a copy of the agenda, powerpoint of information covered, and a sign in sheet for the staff that attended. POC cleared on today's date and provided to Site Supervisor.

To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of punitive nature… 1 Site Supervisor stated a training will be conducted to address children in care personal rights and reporting requirement. A sign in sheet for attendees will be submitted to licensing by 11/08/2024. Based upon staff interviews, staff aggressively grabbed the child’s face and the incident was not address immediately. Due to lack of personal rights this poses as a potential risk to the health and safety of the children in care. 8

No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. 1 Site Supervisor conducted a staff training regarding expectations of supervision with a sign in sheet of all attendees on 09/17/2024. Site Supervisor posted a sign in and sign out sheet before entering and exiting the classroom to remind staff to conduct head count of children in care. Based upon staff interviews, Staff entered the classroom without a child and realized there was a child missing the after door was closed. Due to lack of supervision this poses as an immediate risk to the health and safety of the children in care. 8 Licensee is to provide a written proof indicating as to how supervision will be maintain at all time.
