Based on staff interviews and records reviewed, it was confirmed that supervision was inadequate. 8 Copies of the sign-in sheet will be provided, showing staff names, dates of attendance, and signatures as evidence of participation. Please email Licensing Program Analyst (LPA) Aurelio Mendoza a copy of the sign-in sheet and the training materials by 10/22/2025.
Based on observation, record review and interview, the licensee did not comply with the section cited above in one out of four staff did not Staff #1 could not provide her personnel file; staff #1 is missing the following: personnel record, health screening, Tb test, immuizations, criminal record statement, acknoweldgement to report child abuse, employee rights, verification of education;which poses/posed a potential health, safety or personal rights risk to persons in care.
