Based on staff interviews and records reviewed, it was confirmed that supervision was inadequate. 8 A sign-in sheet will be used during the training. Staff will print their names, sign, and date their attendance. By the end of business on 11/13/2025, the Director will email PDFs of the sign-in sheet and training materials to LPA Aurelio Mendoza as proof of completion.

(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met evidenced by 1 Director agrees to provide a training completed with signed agenda of all staff for reporting duty on incidents that occur at child care centers and reporting responsibilities to CCLD. Plan of correction is due by March 10, 2023. Based on records obtained and interviews from witnesses it was in fact confirmed a child was left outside without supervision which posed a potential health, safety or personal rights risk to persons in care. In addition when the incident occurred staff is no longer employed at this facitlity. 8

Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. 1 The director agrees to watch the "Child Care Reporting Requirements" video on the CCL website and write a statement of what those requirements are. The statement will be submitted with the unusual incident report by the due date of 10/3/22. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Based on observation and record review, the licensee did not comply with the section cited above in Staff #3 did not have criminial background clearance according to guardian roster. LPA confirm with Fresno Regional office the staff had been disassociated on 07/12/22. LPA observed this staff working and caring for children which poses a potential health, safety or personal rights risk to persons in care.

Based on record review the licensee did not comply with the section cited above Infant Teaccher stated she has not been documentating sleeping every 15 minutes with time or initial for staff observing infants sleeping. The toddler component classroom had 3 infants in care with no documentation.which poses a potential health, safety or personal rights risk to persons in care.
