INTERVIEWS WITH THE DIRECTOR AND OWNER WHICH REVEALED MICE WERE RECENTLY SEEN INSIDE OF THE FACILITY. 1 Measures have already been taken to rid the facility of the mice by hiring a rodent proofing company that began treatment on regular basis, beginning 8/21/25. Licensee will submit proof of regular treatments to community care licensing for the next 3-6 months.
Interviews and review of records which revealed on at least one occasion the facility was out of ratio by 2 children when there were 2 teachers and 26 children on the playground for less than 30 minutes Licensee put a plan into place for staff to notify when they are at ratio and have a plan for unexpected arrivals or late or sick staff. Licensee will submit this written plan and a sign in sheet of all staff who attended the meeting by 7/24/25.
a review of records which revealed a child was able to wander into the director's office through the back door from the play yard without the supervising staff knowledge. The child was retrieved by a staff member in the office.
INTERVIEWS WHICH REVEALED DURING THE COVID PANDEMIC PARENTS WERE NOT ALLOWED INTO THE BUILDING FOR HEALTH REASONS.
A TOUR OF THE PLAY GROUND REVEALED THE CUSHIONING UNDER THE PLAY STRUCTURE IS CRACKED AND EXPOSING THE CEMENT/ROCK 1 LICENSEE WILL REPAIR THE CUSHIONING UNDER THE PLAY STRUCTURE ON THE PLAY YARD AND SUBMIT PROOF OF REPAIR TO COMMUNITY CARE LICENSING BY 9/22/23. BENEATH WHICH MAY CAUSE A POTENTIAL TRIPPING HAZARD.
A TOUR OF THE PLAY YARD WHICH REVEALED THE 1 LICENSEE WILL REPAIR OR REPLACE THE STORAGE CABINET AND CLEAN THE PLAY EQUIPMENT AND SUBMIT PROOF OF REPAIR OR REPLACEMENT AND CLEANING TO COMMUNITYCARE LICENSING BY 9/22/23. STORAGE CABINET FOR ART SUPPLIES IS MISSING A DOOR AND THE OTHER DOORS APPEAR TO BE LOOSE, EXPOSING THE PAINTS AND OTHER MATERIALS. THE PLAY KITCHENS ETC. ARE COVERED IN DIRT/DEBRI AND NEED TO BE CLEANED.
