A small cockroach was observed crawling out from/and back underneath a small shelving unit in one of the classrooms. 8 The center is currently on a monthly pest control program and proof was reviewed by LPA during this visit. Center is due for the pest control service for May. Service occurs within the first 2 weeks of the month. This deficiency is cleared today, 4/29/26.

Based on interviews and record review a staff member was written up for grabbing a running child by the collar of their shirt causing the child to fall to the floor which posed an immediate personal rights risk to children in care.

101229 Responsibility for Providing Care and Supervision...(a) The licensee shall provide care and supervision as necessary to meet the children's needs (1)No child(ren) shall be left without the supervision of a teacher at any time,... Supervision shall include visual observation 1 The center will schedule an all staff training regarding supervision. The training will include a plan to ensure all children are supervised at all times, the names of attending staff and the agenda for the meeting. The center will send the agenda of the all staff training and sign in sheet with all attending staffs signatures to CCL by POC date 09/22/25. Director may send agenda and sign in sheet to LPA Mario Caro via email at Mario.Caro@dss.ca.gov. This requirement was not met evidenced by: Based on interviews, the licensee did not comply with the section cited above because it was determined during transition to the outside playground a child was inadvertently left behind in the classroom unsupervised which posed a potential risk to the health, safety or personal rights of children in care. 8

AN IB INVESTIGATION WAS CONDUCTED AND IT WAS FOUND THAT THERE WAS ENOUGH EVIDENCE TO INDICATE THAT A CHILD WAS INAPPROPRIATELY TOUCHED BY AN ADULT MALE WHO WAS AT THE FACILITY TO WORK WITH ANOTHER CHILD. THE DIRECTOR SHALL IMMEDIATELY ENSURE THAT ALL CHILDREN HAVE CONSTANT VISUAL SUPERVISION. ALL STAFF SHALL WATCH THE VIDEO ON SUPERVISON AT https://ccld.childcarevideos.org. STAFF SHALL WRITE A SUMMARY ON WHAT THEY LEARNED AND SUBMIT TO CCL BY 7/25/22. DIRECTOR WAS ALSO NOTIFIED THAT A NON-COMPLIANCE MEETING MAY BE SCHEDULED WITH THE FACILITY BY THE DEPARTMENT.
