This requirement is not met as evidence by: Principal will conduct a staff meeting reviewing personal rights video. Meeting agenda with signatures will be send to LPA by POC date. Based on interviews and staffs' statements, Licensee did not sure that Child #1 was accorded dignity, as staff and child disclosed that Child #1 stated that they were hurt by Staff #4. This is a potential risk to the health, safety, and personal rights of children in care. 8
101223-Personal Rights:(a) The licensee shall ensure that each child is accorded the following personal rights: (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... 1 Per Principal Staff # 1 has been removed indifinately from the facility. Principal will condut a staff meeting reviewing personal rights video and findings of visit. Declarations from staff on what they learned will be submitted. Meeting minutes with signatures will be send to LPA by POC date. This requirement is not met as evidence by Based on records review and interviews it was disclosed that Staff # 1 violated a child's personal rights twice in one day. This is an immediate risk for the health and safety of children in care. 8

Principal will meet with Staff #1 on 04/19/2023 to address incident and provide guidance on how to handle children with care. Staff training agenda and sign in sheet will be provided to LPA Garcia via email by 04/20/23. LPA Garcia observed on this date Staff #1 walking holding child#1’s hand then Staff #1 used force to pull and push child #1 towards the restroom during transition after lunch before nap. Based on LPAs review of documentation and interviews conducted, staff statements obtained during today’s inspection reveal that child #1 was left unsupervised in classroom #2 and was found in the classroom by Staff #1. This poses a potential health and safety risk to children in care. 8 In addition, Per Principal she will have an all staff training on care and supervision and will submit training agenda/sign in sheet via email to LPA Garcia no later than 10/14/2022.
