based on observation, interview, and record reviews, D1 did not comply with the section cited above as a child was left unsupervised in the play yard for about 17 minutes. D1 stated that staff did not follow policy regarding roll calls, which poses an immediate health, safety, or personal rights risk to persons in care. 8 A1 stated the facility has developed a sign in and out form for staff to check children back inside the classroom from the play yard. Deficiency cleared at time of visit.
