Based on record review and interview the facility did not comply with the section cited above due to staff's CPR/First Aid certification cards did not meet 101216(f) requirements which states at least one staff member present possesses current EMSA approved Pediatric CPR/First Aid certifications which poses a potential health, safety or personal rights risk to persons in care.
Based on observation and interview, the playground area has a dirt ground and did not have cushion to absorb falls. This poses an immediate risk to children in care.
Based on observation and interview, Teacher assistant, Staff 2 was observed to be soley caring for 11 preschool children. This poses an immediate risk to children in care.
Based on interview and record review, 8 of 9 staff did not have the immunization against measles and pertussis available for review. This poses a potential safety risk to children in care.
Based on observation and nterview, the Director stated the bathroom on the hallway is being used for the preschool children and staff members. This poses a potential personal rights to chldren in care.
