This requirement is not met evidenced by: 1 Director stated they have provided extra staff to support staff #1 during transition periods. Director also stated classroom #5 has been given a closing teacher so that the classroom no longer has to transition to other classrooms. Based on records reviewed, Directors statements, and the self reported incident, it was revealed that child (C1) was left alone on 10/28/2024, without any adult supervision inside classroom #5 for four minutes which poses an immediate risk to the health, safety and personal rights of children in care. 8

Based on interviews, 3 of 3 interviewed staff confirmed C1 was left alone in classroom #8. This poses an immediate risk to the health, safety and personal rights of children in care.

(1) A result with values of 5.5 part per billion (ppb) or greater shall be deemed an Action Level Exceedance. This requirement was not met evidenced by: Assistant Director states that facility will submit documentation regarding the test results, after outlet D and G are being fixed and retested by due date. Lab report analysis water from outlet D (5.7 ppb) and oulet G (9.7 ppb) taken on 11/05/2022 indicated levels of lead in exceedance. This poses a potential risk to the health, safety and personal rights of children in care. 8
