video review and an Unsual 1 Staff #1 was placed on an indefinite supsension. An emergency staff meeting will be held with all staff to reviews supervision and transitions on 04/25/2022. Meeting minutes and staff signatures will be sent to LPA. Incident Report that was submitted on 04/20/2022. Director reported that on 04/20/2022, child #1 was left alone in the Pre-K 2 classroom for three miniutes after Staff #1 and #2 transitioned 22 other children to the outside playground. This is an immediate risk to children in care if not corrected.

the Unusual Incident Report that the licensee submitted on 10/6/2021. 1 Staff #1 has been terminated as a result of the incident. Facility staff had a meeting following the incident pertaining to supervision and proper protocols. The child's parent was notified of the incident. Staff follow name-to-face protocols when escorting the children to and from classrooms. Licensee reported that Child #1 was left in the Preschool 2 classroom when Staff #1 escorted the rest of the children out of the classroom during a fire drill on 10/06/2021.
