Supervision shall include visual observation. 1 Staff all supervise visually at all times Based on records review, the licensee did not comply with the section cited above. LPA observed C1 in a connecting classroom with no teacher/staff member present which poses an immediate Health, Safety and, Personal Rights risk to persons in care. 8
Per Licensee, Staff #4 was relieved of their duties (due to the incident). Due to the facility relieving Staff #4 from their duties, this poses an a potential health and safety risk to children in care. This requirement is not met as evidenced by LPAs interviews conducted with staff and children, in which disclosures were made that Staff #9 yells and inappropriately handles children in care which poses a potential health, safety, & personal rights risk to children in care
Based on record review, the licensee did not comply with the section cited above in 1 volunteer present and Staff# 3 did not have proof of immunization, which poses/posed a potential health, safety or personal rights risk to the childl in care. 8 one year) and flu vaccination. Flu vacination, faciltiy can provide copy of waiver or proof of immunization. Director agreed to submit via email to LPA no later than 04/21/2023.
