Based on interviews and record review,facility did not ensure that the preschool child was left alone and was able to go inside and use the restroom unattended. 8

Based on Interviews conducted with parents, at least 4 parents mentioned that there child is not being properly accomodated by the teacher in the classroom. This poses a potential health, safety and personal rights risks to children in care. 8 Director must submit the signatures of each teacher no later than 06/30/2022 via email to LPA at: sabina.dodoo@dss.ca.gov

Based on interviews with children and parents. Two children stated witnessing the teacher yelling at a child and 3 parents stated child witnessed teacher taking chair away from a child.This poses an immediate health and safety, and personal rights risks to children in care. 8 LPA will join the meeting via video to observe the meeting.An agenda of the meeting will be emailed to LPA.
