Based on observation, interview and record review, the licensee did not comply with the section cited above in two (2) out of three (3) staff did not have a mandated reporter training certificate on file for review which poses/posed a potential health, safety or personal rights risk to persons in care. This is a repeat violation from inspection dated 9/15/21.

Based on observation, interview and record review, the licensee did not comply with the section cited above in two (2) out of eight (8) medications did not have a prescription label or in the original packing and one (1) medication was expired which poses/posed a potential health, safety or personal rights risk to persons in care. This is a repeat violation from inspection dated 9/15/21.

Based on records review and director’s admission, director did not ensure medication was properly handled and stored as medication for C1 and C2 was expired which poses a potential risk to the health and safety to children in care.

Based on records review and director’s admission Mandated Reporter Training certificates expired for five (5) staff members. This poses a potential health and safety risk to children in care.
