Based on interview and record review, the licensee did not comply with the section cited above in no staff member had a vaild CPR/FA certification on file for review which poses a potential health, safety or personal rights risk to persons in care.
Based on interviews conducted and records review, the licensee did not ensure a safe environment as high levels of lead were detected in a classroom and restroom which posed a potential health and safety risk to children in care. 8 Owner provided CCL with reports from lead testing showing the facility as cleared for occupancy.
Based on observation, interview and record review, the licensee did not comply with the section cited above in one (1) out of two (2) prescription medications did not have label directions as prescribed by the child's physician which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on LPA observation and records review licensee did not ensure medication was properly handled and stored as medication for C1 was not in its original container with prescription which poses a potential risk to the health and safety to children in care. This is an amended version of the report dated 9/8/2021.
