Based on observation, interview, record review, the licensee did not comply with the section cited above by ensuring all staff working/helping with children in care have fingerprint clearance, which poses an immediate health, safety or personal rights risk to persons in care.
Based on observation, interview, record review, the licensee did not comply with the section cited above by ensuring the facility maintains ratio at all times, which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed 1 teacher and 1 aid with 22 children. Shortly after another staff joined the class on the playground.
101229Responsibility for Providing Care and Supervision (a)The licensee shall provide care and supervision as necessary to meet the children's needs(1) No child(ren) shall be left without the supervision of a teacher at any time, Supervision shall include visual observation. 1 SInce the incident, facility has implented a new producure to account for all children before they proceed to the play area. While waiting to be called to go outside, the children sit at their desk, so they are more visible to the staff. The requirement is not met as evidence by: Based on interviews and record review the licensee did not ensure that a child was not left without supervison of a teacher at any time. 8 By 03/15/2022 provide a written statement detailing the changes that were implemented and any future changes to ensure no child will be left unattended.

This requirement was met as evidence by: 1 Facility installed a second gate to keep children inside the deck closer to the room. Submit a written plan of changes that were implemented or will be implemented to ensure all staff have 100% supervision of children at all times. Based on observation and interviews there was a lack of supervision which resulted in a child leaving the premises with another parent. Child was never alone and was returned within 1 minute. This posed a potential risk to the health, safety, and personal rights of children in care. 8 Conduct a staff training on supervision.
