Based on interviews with staff and record 1 All teachers will be retrained on "name to face" in the Brightwheel app to make sure all children are accounted for during transitions. A staff meeting will be held on safety and supervision. S1 was given a written warning with notice of termination if a similar offense occured in the future. review of an unusual incident report submitted the facility did not comply with section above by S1 failing to observe a child wander away from the group after counts were conducted. The child was found in an empty classroom by a parent and brought the child to a staff member.

Based on documentation review and interviews staff could not always clean a 1 Director states that she will revamp the diaper cream form to have specific end dates, and will revamp the potty training/diaper policy to specify procedures. Director will also work on creating a document for teachers that specifies diaper cream use. child properly, and accidental bowel movements were sometimes missed if they were small. This lead to rashes on the skin. Although cream may have been applied with every change as requested, cream was not always recorded. This poses an immediate health or safety risk to persons in care.
