Director to provide LPA with a copy of a new clause in the Parent's handbook that discusses how outside food is handled and diseminted to children while at the facility. This clause will be provided in the 2025-2026 Handbook when school begins again in August. Volunteer provided C1 with a food item containing contents that were documented to cause C1 a non-life threatening allergic reaction. This poses a potential health and safety risk to children in care.

Deficient Practice Statement Based on record review, the licensee did not comply with the section cited above in 4 out of 5 files which poses/posed a potential health, safety or personal rights risk to persons in care.

(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. Director will submit a written plan of correction by POC Date of 05/02/2024. Based observation conducted, LPA observed 18 children left with music staff without fully qualified teachers present in the room which poses an immediate risk to the health, safety and personal rights of children in care. 8 LIC9224 must be signed by all parents currently enrolled. LIC9224 and LIC9099 and 9099D must be provided to all future enrollments for the next 12 months and maintained in file.

Based on interview, LPA learned that one teacher was supervising 18 children while another staff took a bathroom break which places the classroom out of ratio.This poses an immediate risk to the health, safety and personal rights of children in care. 8 LIC9224 must be signed by all parents currently enrolled. LIC9224 and LIC9099 and 9099D must be provided to all future enrollments for the next 12 months and maintained in file.

Based on observation, LPAs observed that a child was behind the slide of the play structure. A staff did went over to the child. There were two staff present, but cannot visually see the child from where they were positioned. This poses a potential health and safety risk to children.

Director states that faclity no longer uses those fountains, has them sealed off from use. Facility will contact water program for remidiation. Facility to send plans and completion of remidiation by 11/28/2022 This exceedance level found in “B” drinking fountain was 8.1 ppb and drinking fountain "E" was 20 ppb. This poses a potential risk to the Health, Safety, or Personal Rights of children in care.
