(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher...Supervision shall include visual observation. Campus Director and staff confirmed new procedures in place to ensure children are supervised and accounted for all times. Staff will be completing a census count/head count as morning meeting begins using the attendance sheet as a guide. This requirement is not met as evidence by, per LPA Laureano's reviewed of the Unusual Incident Report received, and interviews completed, it is confirmed that facility left child without visual supervision on 4/30/2025 for approximately 10 minutes, which poses an immediate health and safety risks to persons in care. 8 When children use the outdoor preschool bathroom, a teacher will be responsible for opening and closing the bathroom door and confirming no children are left behind.
