During staff file reviews, LPA observed mandated reporter certifications for 3 out of 5 staff were expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. LPA discovered 11 infant children and 13 toddler age children were missing the LIC 9224 Acknowledgement of Receipt of Licensing Report dated 4/12/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.

Director took immediate action by placing S1 on administrative leave to conduct an internal investigation, contacting families, authorities, and licensing. S1 was terminated 03/10/2023. All staff received additional training including mandated reporter. LPA interviews with Director and four staff members corroborated S1 being verbally and physically aggressive toward the children in care. This poses a potential risk of personal rights to the children in care.

(1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. 1 Director agreed to conduct a training for the staff in relation to Responsibility for Providing Care and Supervision and will watch the Department Training video related to Providing Care and Supervision on the Department website at www.ccld.ca.gov. A sign in sheet will be provided to the Department related to the training discussion and the attendees by the due date 4/19/23. Based on LPA staff interviews and statements, it was revealed that child (C1) was left on the play yard on 4/10/2023 without any staff supervision. interview with two staff who indicated C1 fell and hit head on bookshelf but medical 1 Licensee will have staff trained on head injuries and what to look for as symptoms for possible concussions. Copy of trainings and proof staff took training by plan of correction date personel were not notified and furthermore throughout the day child began to vomit and had fever and was lethargic. Interview with parent indicated they were not notified about head injury until 2 days later. This poses a potential risk to clients in care
