Responsibility for Providing Care and Supervision (a) (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced 1 Site Supervisor provided documentation proof that an immediate training regarding active supervision, staff responsibilities, and updated procedures for keep account of children present, along with staff attendance sign in sheet, which was completed on this date. Site Supervisor stated she will also by interview and observation of video footage. Information obtained indicated that child #1 was left in the outdoor play area unsupervised for approximately3 minutes. This poses an immediate risk to the health, safety or personal rights of children in care. A civil penalty of $500 was assessed. 8 continue to monitor all staff and enforce these new procedures in the classroom and during outdoor play, to ensure staff understand supervision requirements, children’s personal rights, and health and safety…
Site Supervisor stated a training will be conducted on 10/13/2025 regarding active supervision with all staff and will provide sign-in sheet of attendees to Licenisng by 10/16/2025. Based upon staff interviews, Staff entered the classroom without a child and realized there was a child missing after entering another classroom. This violation poses a potential risk to the health, safety, and personal rights of children in care.
