Based on LPA's record review of staff time cards and children sign in/out sheets. 1 Director will submit a written plan of correction no later than the end of business day on 11/8/2022 explaining how the facility will remain within required ratios at all times. 1 teacher was found supervising more than 12 children on two separate days. Facility was operating beyond staffing ratios. This is a type A Deficiency which poses an immediate Health and Safety, and personal rights risk to persons in care.
