101700.3 California Lead Action Level at Child Care Centers (b) Testing results with fractional ppb readings of 5.0 ppb or greater shall be rounded up to the nearest whole number, before comparing to the Action Level. (1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. 1 LPA observed Site ID A located outside classroom by children’s restrooms with a sign that reads "Out of Order" Children in care bring their own individual water bottles from home. Facility also provides drinking water for children in care in the means of drinking faucets and refillable water stations which have passed lead testing.Facility will re-test on March 21, 2023 and submit results to LPA Duron. Based on lab report analysis water from outlet A (20.000 parts per billion) taken on 1/24/2023 indicated levels of lead in exceedance. Staff stated facility is scheduled to re-test outlet on March 21, 2023. 8

Based on LPA's physical plant inspection The head teacher stated they will discuss the violation with director when they return and will send out a notice to all staff to remind them of the CDPH covid19 Guidance, and face covering Guidance. Head teacher stated that staff will sign a form to be submitted to LPA, demonstrating that all staff in the facility understand, acknowledge, and will follow of the classrooms, it was observed that 2 of the 8 staff were either not wearing a face covering at all or not wearing the face covering properly. This constitutes a potential threat to the children's health, safety, and personal rights. 8 this Guidance. Proof of this will be submitted to LPA by 11/16/2021.

Head teacher stated that this notice of the classrooms, it was observed that 0 of the 53 children were either not wearing a face covering at all. Based on this, LPA has determined children are not being encouraged to wear face coverings while in care indoors. This constitutes a potential threat to the children's health, safety, and personal rights. 8 and proof that parents were informed of the above items listed, will be submitted to LPA by 11/16/2021.

Director will submit verification of training to the Licensing Department via email. This requirement was not met as evidence by: The facility does not require children ages Director states she will send out communication to the families and train staff regarding the requirement masks for children ages 2 and older. Director will email the communication to LPA Hawkins by POC date. 2 and older to wear masks while in the facility as required by the CA Dept. of Public Health Guidance on the use of face coverings on children issued June 18,2020 and updated November 16, 2020 and an individual mask exception did not apply. This is a potential risk to the health and safety of children in care. 8
