Based on information obtained, interviews and video footage obtained C#1 was was obsered to be outside of the facility premises. Staff did not see the C#1 exit the facility. 8 3/20/24. Director is having building inspected to have the gate reassessed to determine what nees to be done. They will also increase the loudness of the alarm and add convex mirrors.
Responsibility for Providing Care and Supervision--(a)The licensee shall provide care and supervision as necessary to meet the children's needs. etc... This requirement was not met as evidence by... 1 Per Director she will have a staff meeting to discuss child’s personal rights and will watch personal rights video on https://ccld.childcarevideos.org/ Director and staff will write a declaration with plans implemented for this not to happen again. Declarations will be submitted to LPA via email by POC date. Based on staff interviews and video footage, child#1 was found outside the facility by S#1 in the courtyarf of the facility which has public access. Staff did not see how child#1 wonder off the premises. This poses an immediate health, safety and personal right risk to the children in care. 8 of 3/20/24 along with summaries of what they learned. Director stated that she will have someone near the gate at all times when the children are outside and add convex mirrors.
Unusual incident report that was submitted to the Department on 04/24/23. An unusual incident occurred on 04/03/23 and was not reported to the department within the required 24 hours. This is a potential risk to the health and safety of children in care. This is a repeat violation within 12 months. Civil penalties are being assessed.
record review. 1 Interim Director states she will make sure that the facility is in ratio at all times. They are hiring an additional teacher that is set to start on May 1, 2023. it was observed that on February 24, 2023 at approximately 9:52 AM there were 14 children present in the Preschool classroom with one teacher and on March 15, 2023 at approximately 10:00 AM there were 14 children present in the Preschool classroom with one teacher; these instances placed the facility out of ratio. This is an immidiate risk to the health and safety of children in care.
staff disclosures. On March 15, 2023, a staff member placed a shelf in front of an exit door in an attempt to prevent children from leaving the classroom. Disclosures were made by staff confirming this incident. This was a immediate risk to the health and safety of children in care. 8 Interim Director states that shelves will not be placed in front of doorways. The facility plans to reccomend to the building that door alarms will be placed on the doors. Written statement will be sent to LPA by 04/17/23.
review of Criminal Record Clearances 1 LPA spoke to Director, Michelle Johnson, via telephone. Director states she is trying to associate both staff to the facility at this time.

facility file review and disclosure by the AD. Incident #1 occurred on 07/13/22 and reported to the Department on 08/05/22. The facility became aware of the incident on 07/15/22; Incident #2 occurred on 07/29/22 and was reported to the Department on 08/05/22 via phone. The facility was required to report both incidents to the Department within 24 hours and failed to do so. This is a potential risk to the health and safety of children in care.
Based on licensing staff observation, the licensee did not comply with the section cited above in 1 out of 1, Licensing staff observed that there is a broken light cover that is accessible on the outdoor play graound. Licensing staff also observed pieces of wood on the yard which are not being used., which poses/posed a potential health, safety or personal rights risk to persons in care.
