This requirement was not met evidenced by: 1 Per Director, teacher completed respectful redirection training over the weekend and provided certificate. Per director, she will review challenging behavior trainings during monthly in service. Based on interview, teacher made threat to child due to behavior issues, which poses a potential health and safety risk to children in care. 8

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met evidenced by: 1 Per director, immediate practice of extra transition protocols of counting children before moving are being re-inforced. Per director, additonal supervision and counting training is going to be implemented into the professional development on 2/21/25. Based on interviews and record review, one child was left outside in the play yard alone without adult supervision for 15-20 minutes, which poses an immediate risk to the health and safety of children in care. 8 Director provided LPA of supervision and counting protocols during today's visit. Per Director, she will email to LPA copy of meeting agenda and staff sign in when training has conducted.

101215.1(h)(1)(a) child care center qualifications, three of the 15 units required in (h) (1) above shall be in administration or staff relations. This requirement was not met evidenced by: 1 Per assistant director, she is going to enroll in administration course as soon as possible. During visit, LPA observed assistant director search PCC and GCC for course. Per assistant director, both colleges are not offering the admin course during the spring 2025 semester. Based on interview and record review, assistant director is missing 3 units of administration or staff relations, which poses a potential health and safety risk to children in care. 8 LPA suggested checking Laguna Tech and University of Arizona online. Assistant director will confirm enrollment of course to LPA by email registration confirmation by 2/25/2025.

Outdoor activity space §101238.2(d)(2)
(d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement was not met evidenced by: LIcensee stated she will block off the area , gardener will clean every week to put the sand back in the sand area .License stated teachers will clean sand pit area to make sure no sand on the pavement .Licensee will send the pictures of correction to the LPA by POC date . Multiple incdents were recorded in the area where uneven pavement and sand pit . LPA observed incident reports in the children`s files and verified with staff place of the incident. Per staff uneven pavement is the area where most of the incidents happened. Disclosures were made that it could be at least 1-3 incidents per week . 8
