Based on interview and record review, the licensee did not comply with the section cited above as children's files did not have a signed LIC 627 Consent for Medical Treatment form. The facility has an in-house form that is similar but does not have the same statement as the LIC 627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interviews, the licensee did not comply with the section cited above as 2 staff members stated an infant was left unattended in the outdoor play area on the morning of 08/22/2024, which poses an immediate health, safety or personal rights risk to persons in care.

Licensee will ensure that all staff are thoroughly trained on record keeping requirements for safe sleep. Licensee will create a written statement with all requirements for documenting 15-minute checks on sleeping infants and a plan for how staff will meet these requirements. During record review, LPA Uribe found that staff have not been meeting the requirement for documenting 15-minute checks on sleeping infants which poses a potential risk to the health, safety, or personal rights to children in care. 8 Licensee will have all staff sign and date this written statement acknowledging their responsibility to maintain compliance with this regulation. Licensee will email a scanned copy of this signed written statement to LPA Uribe at christina.uribe@dss.ca.gov no later than the due date of 09/06/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as 1 infant (under 12 months of age) was sleeping with a sleep sack on during nap time. which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above as 0 of the 3 staff members present during today's inspection is a fully qualified infant teacher, thus the facility is out-of-ratio, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observe that facility did not have the 15 minute check log, Director stated that facility visually check the children, but did not know that it needed to be logged on paper.

Based on LPA observation, the licensee did not comply with the section cited above. LPA observed an infant bouncer and inclined sleeper present in the infant classroom during today's visit. This posed a potential health, safety or personal rights risk to persons in care.
