Based on LPA's observation and record review, C5’s medication is stored without the child's name identified on the outside of the medication and medication was stored without the product label. Additionally, C5’s medication was without administration instructions including the incidental medical service and no policy was located with the medication, in the child’s file or provided to the department which poses/posed a potential health, safety or personal rights risk to persons in care.
