Based on interviews with staff and children, the facility failed to comply with the Personal Rights regulation. Children appeared hot and uncomfortable, with moist faces and signs of irritability which posed a potential health, safety or personal risk to persons in care.
Based on the facility surveillance video footage and interviews it was determined on 02/06/2025, a staff handled the day care children in a rough manner. This poses an immediate health and safety risk to the clients in care. 8 The staff will watch and write a summary on the following supervision videos: Active Supervision and Outside Play - YouTube, Active Supervision Keeps Kids Safe - YouTube and https://mybrightwheel.com/blog/active-supervision due to LPA no later than 03/20/2025.
101216.2 (e) Teacher Aide Qualifications and Duties. An aide shall work only under the direct supervision of a teacher. This requirement was not met as evidence by: 1 The director stated she will ensure the staff will be educated with the staff qualifcations. The director will create a written plan of action for staff to follow if there is no qualified aide available to assistant. The former preschool/director was allowing an unqualified teacher's aide alone in the classroom with up to 15 children at one time. This is a potential health and safety risk to the children in care. 8 The director will have the POC submitted to the LPA no later than 04/10/2025.
Based on licensees acknowledgment and the facility playground tour. On 11/07/2024, LPA Hood and the licensee observed an off-limit area that was not made inaccessible to the children in care. This area contained two metal poles, cement brick This is a potential health & safety risk to children in care.
Based on the licensees admittance and the LPA's observation. It was determined the temporary fencing around the off-limit area on the playground was not in good repair before allowing children access. This is a potential health and safety risk to children in care.
