Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care.

All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1)Obtain a California clearance or a criminal record exemption as required by the Department or This is requirement is not met as evident by: 1 By POC 07/15/2022, Site Director stated that she will submit paperwork for S-2 and understands that S-2 cannot be at the facility until fingerprints are cleared and associated. Based on record review, S-2 completed livescan on 06/29/2022; however, facility number was entered incorrectly. S-2's fingerprints are not cleared and associated to facility, which poses an immediate health and safety risk to children in care. 8

Building and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evident by: 1 Defiency has been corrected. On 05/27/2022, facility had their contractor move the wooden beam higher. It is currently about 6 feet off the ground. Based on observation and record reviews, facility had a wooden beam that went across the pergola that was about 3 feet off the ground. LPA observed that children had to bend down or squat down to go under. There was an incident where a child bumped into it and obtained a red mark on their face, which poses a potential health and safety risk to children in care. 8
