Based on interviews and record reviews, C1 violated C2's personal rights as a result of inappropriate behavior which occurred on 01/27/2026.
Based on observation and interviews, CCC staff had reasonable suspicion that the incident occurred on 02/03/2026 and was not reported to CCLD until 02/06/2026. This poses a potential health, safety, or personal rights risk to persons in care.
Based on observation, the licensee did not comply with the section cited above in that 10 preschool children (ages 2 to 3) were being cared for in Rooms 1 & 2. LPA notes Rooms 1 & 2 are not licensed for preschool children. Additionally, CCLD was not notified of the changes. This poses a potential health, safety or personal rights risk to persons in care.
(a) The licensee shall provide care and supervision ... to meet the children's needs. (1) No child(ren) shall be left without the supervision ... shall include visual observation. This requirement was not met evidence by: 1 Facility director will submit a written plan of correction (POC) stating measures to be taken in order to follow Title 22 Regulations and avoid another incident like this happening again. Director advised an informal conference will be scheduled on November 16, 2021 at 10:00 AM. Directors POC will be due on that day. Based on LPAs observations, interviews, and records. The facility failed to supervise a child resulting in an Absence of Supervision in another playground for 24 minutes. This poses an immediate Health and Safety risk to persons/children in care. 8
