No child(ren) shall be left without the supervision of a teacher at any time, except as specified... Supervision shall include visual observation. This requirement is not met 1 Per lead teacher Esther Villa, a plan of action regarding procedures for providing adequate supervision will be created for all staff to review and all staff will provide a written decleration of the knowledge obtained from the meeting held. Proof of meeting along with meeting materials will be sent to LPA via email by POC date above. as evidence by: Interviews conducted disclosed that S1 and S2 were not providing adequate supervision and as a result a child sustained injuries, resulting in permanent scarring. This possesses an immediate risk to the health and safety of children in care. 8
