Staff are not properly trained to meet day care child's medical needs.

Per Director, additonal training was provided to Classroom #2 staff. Staff did not prevent Child's finger from getting wedged between the door at which point their finger was pinched requiring medical attendtion. 8 POC cleared during visit. LPA obtained a copy of the signed training.

Based on record review, the licensee did not comply with the section cited above in Staff #1, 2, 3, 5 and 6 do not have current certificate which oses/posed a potential health, safety or personal rights risk to persons in care.

Staff #2 was hired as a teacher, but the transcripts are from a foreign country and has not been evaluated by an approved agency. This poses a potential risk to the health and safety of children in care.

Based on record review, the Director did not comply with the directive above, as two outlets (outlet used for food preparation in the kitchen) and (outside water fountain outlet) out of five outlets tested with an Action Level Exceedance (ALE). This poses an immediate Health and Safety risk to children in care.
