This requirement was not met by... 1 Facility ceased operations on 01/22/24 after finding out they could not operate due to their license not being granted final approval Facility became license on March 7, 2024. Based on interviews and record review, it was disclosed the facility started operating in January 8, 2024 for a period of two weeks until January 22, 2024. This poses an immediate health and safety to children in care. 8

During inspection LPA observed child being picked up by his mother. Director agrees to come up with a plan to avoid commingling children in the future by 04/19/2024 During inspection LPA observed one Preschool Age Child to be commingling with four toddlers in the toddler component room which poses/posed a potential health, safety or personal rights risk to persons in care.
