Based on records reviewed and 1 The director stated the facility is to provide a statement to LPA why the LIC9224 was not provided to parents. Facility is to provide copy of LIC9099 dated 02/15/24 that documents any Type A citation to parents/guardians of all children currently enrolled by the next businsess day or the next day interview, The facility did not provide the 02/15/24 licensing report LIC9099 and acknowledgement receipt LIC9224 to all infant families by the next business day. this poses an immediate risk to the health, safety, or personal rights of children in care. 8 children are in care, and to any newly enrolled parents/guardians for 12 months from the 2/15/24. Facility is to provide signed LIC9224 to LPA by 5/9/24. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must also be placed in the child's file for verification.

Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights:.. (3) To be free from corporal or unusual punishment, infliction of pain... actions of a punitive nature including but not limited to: interference with functions of daily living including eating... The requirement is not met as evidence by: The director stated that the facility conducted a staff training about personal rights, supervision, napping, feeding and etc. A copy of the training was given to LPA A staff restrained a child’s body and arms by using right arm in order to force feed child. This poses an immediate risk to the safety of children in care. 8

(f) The items specified in (d)(1)(A) through (H) above shall also be reported to the child's authorized representative. This evidence was met by: 1 The facility stated that a statement stating a meeting and communication with the preschool director Ms Bongcayao and school director Ms King on 9/14/23 and 9/15/23 respectively was conducted. Based on interview, there is no document showing that the child's representative were notified about the incident between the staff and the child during feeding. This posses as a potential right to saftey of children in care. 8
