Based on observation and interviews, 11 preschool children were observed on the desginated Toddler Yard. The Toddler yard is not licensed for preschool children.This poses a potential health and safety risk to children in care. 8 The toddler yard will be reverted back to its approved and licensed use. Director agrees to submit updated facility sketch identifying rooms and outdoor play areas per component, Off limit areas and classroom names.

Based on LPAs' interview, observation and record review, the facility did not comply with the section cited above, At 9:41 amd LPAs observed 7 children on the second level of the Harbor building and at 11:51 a.m. LPAs obervered 12 children in Upper room D on secondd level of the D building. Harbor Building and the second level of the D building were not approved for licensure, which poses/posed a potential health, safety or personal rights risk to persons in care.

This requirement was not met as evidence by the Action Level Exceedance results revealed 1 Director has sealed the water outlets. Outlets are not use for food or drinking consumption. Plan to remove the outlets completely by 8/01/2023. Director will send LPA a Plan of Correction regarding remediation of outlets on or before 8/10/2023. that levels of lead were beyond allowable amount deemed by the State of California and if not corrected this poses a potential Health and Safety or Personal Rights risk to children in care. 8

Licensee agreed to conduct in service training on supervision and provide agenda and list of staff in attendance to LPA by 9/20/2022, via email. All staff shall view supervision of Children Video on CCLD website. Based on observations, interviews and record review the licensee did not ensure children were provided with care and supervision to meet the children needs.
