101238 (g) (2) Buildings and Grounds: (m) Firearms and other weapons shall not be allowed on or stored on the premises of a child care center. The requirement is not met as eveidenced. Per Lead Teacher and Director of Plant of operation, facility will submit a firearms waiver no later than 09/16/2022. LPA provided guidance to Lead Teacher about the process of firearms waiver and declaration was obtained. On 9/15/2022 Upon arrival, LPA observed two securities guard with firearms. Per Director of Plant and Lead Teacher, the facility did not report to DSS and do not have a firearms waiver on file. This poses an immediately health & safety risk to the children in care. 8

101238. a. Building and Grounds (a) The Child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement not met as evidence by: 1 Per Lead Teacher and Director of Plant operationsl followed Written Directives in PIN 21-21 CCP for lead water testing. Licensing Forms completed; LIC9275, LIC9276 and facility sketch was revised and provided a copy to LPA. LPA obtained a LIC855 Declaration from Lead Teacher regarding the plan of correction of the two areas that exceed the lead results. The faucet from the kitchen sink will be replace on 09/19/2022 and facility will contact the sampler for re-test. Facility will submit a new report to LPA Miranda no later than 09/30/2022. Based on lab report analysis water from the D – Drinking Fountain – Hallway ( 7.5 results Ug/L), and B – Sink_Faucet_Kitchen (5.8 result per Ug/L.
