Based on observation, interview and record review, the facility did not ensure to maintain a level to be under 5 ppb at fixtures B and D. This poses a potential health, safety or personal rights risk to persons in care. 8 Facility must have running water for Faucet B and D until fixtures are retested with a certified water sampler by 03/01/2024. Once retest is done, faciity representative will submit retest results to the department.
