Based on interview and record review, Site Supervisor informed LPA that on 12/23/25, C1 was left unattended in a classroom for around 3 minutes while other children were escorted to the bathroom. by the close of business day (5:00 PM). This written plan of correction will detail how such occurences will be avoided moving forward. Pictures of returned LIC 9224's should also be sent to LPA before the end of the week (1/9/26).
Based on observation, interview and record review, the licensee did not comply with the section cited above as Site Supervisor (expiration 11/20/21) and Administrator's(expirations 1/20/22) certifications were not current which poses a potential health, safety or personal rights risk to persons in care.
